has there ever been a case in the history of the site where the SC wrongly disqualified someone, and then after the disqualification undisqualified the person?

I know they reversed a suspension from the site after the suspendee presented additional evidence. I think the SC has a clear understanding of the seriousness of a DQ and makes every effort to be sure they're on firm grounds before issuing it. The SC members don't always agree on a DQ, but do their best to reach a consensus before taking action. That can be agonizing if it is your photo "on the bubble" so to speak.
