This is a fantastic, very bold photo.
Of course, it's not just the beginning of emphysema, but a lot of other health problems (actually, we can only assume that the person is smoking by the hand position as if holding a cigarette. It would really be wow if there was some smoke visible.

Black is commonly used to symbolize death. In this image the smoker, even dressed in black(death) with a foot in the grave, has barely any life(light) left, and what ever light there is they are using to close that coffin, by smoking not even looking into the source of light to get out but staring into the black void...

or its the first thing that popped in his head since he has to title his challenge entries now...

I think this is such a powerful image! I thought it was PERFECT - glad to see that FINALLY you are gaining the 'mainstream' recognition for your images.

Remarks: Mais oui! God bless the spot meter! -Despite the dramatically minimal view and composition, I imagine an even more satifying image - either less static or less featureless... As it is, with a back turned at me, it feels (just a tad) \"stolen'. Hell, it's still a poise with magical balances and tension, from subtle to bold, which would be all the more prolific in an image twice the size (or a 20x30).

I'm sorry, I get how the title meets the challenge, but I don't get how the title matches the photo. It doesn't convey shortness of breath, or asphyxiated lungs, or lung damage, or anything that is associated with Emphysema. While it is a nice picture overall, I don't feel it really meets the challenge.
