I like the glowing in this, to me it indicates a visualization of the pain he is feeling.

What happened to the detail of the index and middle finger of the right hand and the knuckles of the left? Looks like it was dodged out...

Nice detail, and I guess you know about the haloing and chose to use it as a style? I like the perspective and the excellent dof. The haloing not so much.

Way to much haloing around the hands takes away from the shot. Nice use of DOF though to focus the attention on the thumb and needle.
