The title is strange. Is it a spelling mistake?

The lighting highlight the chest more than it highlights the happy face, which seems to be the focus of the shot, according to the title. Maybe a different black and white conversion would have fixed that by lightening the happy face and darkening the skin.

I think I would have framed this shot differently, with all of the in-focus tomato in the frame and less of the not-fully-in-focus tomato cropped out a bit more.

I agree that this has an "old" feel to it. The boy even looks like he came from the 1920s or so. For some reason, his expression reminds me of what Shirley Temple looked like when she sang : ) I don't mind the darkness too much since his eyes still bring you in. Maybe cropping out some of the background (which is kind of dark) would help even things out a bit too. Nice shot.
