Great idea! Important message in a very creative manner! But 2 things are disturbing the impression a little bit.The left side of the injection is a little overexposed because of the strong direct illumination. And I would have cropped a little bit different. IMHO there is too much 'empty space' below your main subject. Using the 'rule of thirds' could be helpful here. Nevertheless I like your shot and voted 8. -nds
