Since we are allowed to remove things such as power lines and etc in our photos, is it allowed to clean up fingernails and etc in a portrait? Would that be considered distracting elements?

If you mean, run a virtual nail file on them to clean up grime, that should be fine. If you mean, virtually trim 3-inch nails to normal nails, that would depend on their prominence. You could put polish on them with impunity, pretty much, if you stayed with the actual, defined shape, but you couldn't give them a French style (white tips) if they didn't already have one.

YW. Kids DO get grubby, don't they? Part of their charm, actually, according to me :-)
