that is what i tried to do in this one but i guess i over did it there too, thanks for the suggestions and tips

Yup - that's a whole lot better than the original 'AFTER' image you posted. It could be debatable about the liverspot on the face though - that sort of thing becomes quite a delicate subject to broach with the person :) For example, my wife has a mole on her nose. If I take it out of pictures, it doesn't look like her any more. But spots and blemishes get removed. The teeth are probably unaturally white too. But it is certainly a lot more like the person than the first attempt so you are heading the right way.
Think the key is to be subtle. Most 40 year olds don't want to see a picture of what they could look like if they were 20 - it is just upseting, unrealistic and obvious. But a picture that shows them looking great as they think they look, in their minds eye and isn't too far from what they see in the mirror is a big difference.
You might bring the teeth back to more like the tone they originally were, but maybe slightly brighter.
Also the neck folds could be lightened a touch - again not removing
them, but making them less noticeable.
The back lit wisps of hair on the left of the face could be removed.
Maybe slightly, and subtly lighten the entire eye sockets to move focus there.
I did this a while ago and would probably do even less editing now, but
this shot has had about 40 separate changes. But it still has skin
textures and a mole or two.
