 Used two overlays from here.

multiply, overlay and hard light blending modes at varying opacities.

adjustment layers in levels, brightness/contrast, color balance, dodge & burn, and gradient map to desaturate colors

The processing on this is wonderful. Really love the tonal range and the blurring is great.

Lensbaby was really a great choice for this - nice old feel to everything in the shot, as well as the texture.
