 In RAW: Boosted Highlights +100, Lights +5, Darks +15, Shadows +5.

In CS3: Increased brightness. Created layer mask for dodge and burn technique. Filter > Stylize > Find Edges to create new mask and used Glaussian blur .05 and USM tool. Cropped photo.
