Some people use a syringe sans needle to administer drugs intra-nasally. I don't think it's very comfortable, but it does lower the risk of contracting AIDS, syphilis, hepatitis, endocarditis, or necrotizing fasciitis. |

Maybe a little too-brilliantly lighted...I think I'm expecting it to look dimmer and softer, as if from candles or lamplight. You'd risk lose some detail, but might add to the mood.
You might try using a tone curve on adjustment layer to add a yellowish cast without losing the contrast and detail. |

The syringe should have a needle if it's too be effective. Also the 'coke' is a little TOO spread about in my opinion...
We need a focal point too. What are we supposed to look at the most? |

I think it could do with a strong focal point, my eye's wandering too much. Nice off-white on the drugs, it's a detail most people would not catch. ;) |
