 Almost straight out of the camera! This was shot at f/3.5 for very shallow DOF focusing on the model's fingertips. The rest of the blur is natural lens blur.

I'm wondering about people's reaction to the blur on her shoulder ... if they might think that is an editing aberration. But, as I said, this is practically straight out of the camera. I processed via Lightroom (doing almost nothing to it), then reduced the size and added a border using Photoshop CS4.
