 used jpeg file, first in camera raw, then in cs5.
fixed a few hairs out of the nose, enhanced image with dodge and burning tools.
curve mask in the eye area.
b&w layer, with 90% opacity.

I feel that you lost much of the emotive power of this image by putting the nose in focus and having the eye soft. Reverse that and its a much better image.
