Thanks for all the comments. Unfortunately, I focused too much on the setup than the set and my score reflected it. I created the shot using two mirrors in front of one another, the front mirror was broken (packing tape applied behind it surrounding the 'hole' to keep the pieces together). I then just took a picture of myself holding the mirror at about a 45 degree angle relative to the camera lens.
Things I could've done better:
1) USED A BETTER MODEL!
2) 'flared' the front mirror shards more to improve the distortion effect
3) USED A BETTER MODEL!

You are so right!! You should have used a better model. Something from the lake perhaps.

Your skin tones seem to be a little to pinkish. I hate trying to work with those tones because they have so many elements that are hard to balance out...

Skin is awfully pink! Against the B&W of the rest of the picture, it looks almost unnatural.

very interesting shot. the skin tone seems oddly pink against the gray, might have played with adjusting it a little to make it look more natural but otherwise a very nice picture.
